The two barometers doctors use are biopsy and Viral Load test. That test tells how active and intense the virus is. Some people keep a lower level under a million, mine was 14 million and some doctors tell people with very low viral loads they can wait and decide later possibly. People with high viral loads I would think always get suggested biopsies.
It is blood borne only, infection must be from a blood source to a blood source, that is why it is the norm that partners do not have it, in spite of intimacy. In normal activities there must be a contact medium sores etc.
