I bought TTB's Live From Oakland recently on 3LP (already owned the 2CD/DVD combo) and i wonder if anybody experienced this same issue: it came with a card for a free digital download of the record but i can't access the website: www.memphisvinyl.com/downloads. While googling memphis vinyl i only found their website where you can place orders to have your vinyl records pressed at their factory.

Just contact their customer service, it might take a few days for them to get back to you, but they should take care of it. I had a problem once with a DL and they were really great to deal with.
